Bareboat Chartering Certification (ASA 104 or Equivalent)

I hereby request my cruise check out in Fairwind Yacht Club. In order to qualify for cruise check out, I affirm:

I am a member in good standing of Fairwind Yacht Club at the medium or large boat level.
I have been trained and checked out on one or more medium/large boats.
I have achieved certification in either ASA 104 Bareboat Chartering, US SAILING Bareboat Cruising, or another certification reviewed by the Fleet Captain and approved by the FYC board.

Final Confirmations

I understand that I will be required to complete and submit a float plan for any time I am keeping a Club boat out of the slip overnight. I understand that I will be able to have a maximum of one overnight reservation in place at a time in addition to a maximum of one day sail reservation at a time. I understand that the Fleet Captain must approve this request before I am cruise checked out.
